Happy Tuesday! A great way to challenge school age children is by using a T-stool. It's is a stool with a single leg that can be adjusted for height. Static sitting is a small challenge, and trunk rotation, combined with throwing and catching a ball thrown on and off center can be a HUGE challenge!! Happy Monday! It can sometimes take years to fully appreciate the fruits of your labor. I just experienced this feeling first hand. I was going through old treatment pictures and videos and came across one of baby Liba. I began with her just over 2 years ago. She was doing NOTHING! Minimal head control, no trunk control, no stability or mobility skills. A short time later, she began to perform a modified commando crawl. The pattern was so strange and out of the ordinary that I shot a quick video. After tons of hard work using #cuevasmedek exercises, Liba began to catch up. She is now 3 years old and is about to age out of EI. The videos of her jumping were taken this week! Happy Wednesday! Tandem walking means walking with one foot in front of the other in a heel to toe pattern. It is challenging because your child has to walk with a narrow base of support. A great way to practice it is on a balance beam. Encourage your child to keep their feet straight, rather than twisting sideways to widen their base of support to make the activity easier. Tip of the day!! If you enjoy exercise, chances are you have a physio ball in your home. (If not, they are pretty cheap online). Good news! It is a tool that many Physical Therapists have been using to treat kids. Try this at home. Sit or kneel on the floor and sit your baby/toddler on the ball facing you. Hold their pelvis or lower torso and slowly tilt the ball 45 degrees in each direction, then return to center. If your child has good control, they should be tilting their trunk, trying to come back to center when you tilt them. Happy Tuesday! Why are some kids athletic and others can't seem to achieve even basic sports skills? A lot comes down to natural abilities, but there are things you can do to point them in the right direction! 1. Work on core strength from 3 months and on - We need proximal stability before distal mobility. In other words, if your core is strong, all activities which require your hands or feet will be easier. 2. Play with your kids consistently- Sports can be broken into what we call splinter skills. These are the smallest components of a larger skill, ie, picking up your arm to throw properly. By playing with your child, you will be a dressing splinter skills that apply to numerous sports activities. 3. Find activities your child enjoys- Coordination can be tough for some kids. The more your child enjoys something, the easier it will be to motivate them. So many skills overlap to various sports, so don't worry!! 4. Shocking sonogram at the OB-GYN office: I saw a friend last night who told me that at a recent visit to the OB office for a routine sonogram, his niece was given the news that her baby has Spina Bifida, a serious malformation of the spinal column that can have lifelong implications. The family is shocked, sad, crushed. So many questions...so much is unknown. I sat him down and told him all I knew from a clinical perspective. In my 20+years of practice, I have worked with many children with Spina Bifida. Although that pain is still so fresh, he left with information on the condition and whom he could speak with to help his family prepare for this baby, and most importantly, he left with HOPE! Tip Of The DAY! Happy Wednesday! It seems every summer I get a similar call. "My son broke his arm in camp. Does he need Physical Therapy?" The quick answer is generally not, but maybe. It depends on a few factors. Firstly, if a joint such as the elbow, knee, or ankle have been casted and thus immobilized, it may be stiff when the cast comes off. Second, the longer the cast is on, the more the muscles can atrophy, or shrink. It's very much a use it or lose it kind of thing. Lastly, the severity of the break matters. So when the cast comes off, soak the joint in a warm bath, then move the joint gently. If it hurts, stop. The range of motion and strength should return within a few weeks. Tip Of The DAY!! Happy Monday everyone 😊 Kneeling (left picture) and half kneeling (right) typically start at about 9-10 months. Kids crawl and as they try to stand, get into a kneeling position and pull up. This bring us to 2 important developmental points. Firstly, when babies refine their ability to stand, they use half kneeling to stand. This ability of having 1 leg forward and 1 leg behind is called dissociation. Not only is it more mature, it also prepares the child for smoother movement transitions and many more play opportunities! Second, playing with your child when in kneeling & half kneeling positions, makes them stabilize their core muscles and build strength. If your child can't do it by a year old, there's a strong chance they have trunk/core weakness. Happy Friday friends!!❤ Coordination skills are a complicated and beautiful harmony and cooperation of the brain and the body working together to create fluid movement and achieve tasks. However, when we look deeper, these skills can be broken down into sub-skills. Reaching, hand- eye & foot-eye coordination, visual tracking, visual convergence, reflexes, & speed are just a few of the sub-skills. But what should you do if your child is having a hard time throwing and catching a ball? A great way to practice these skills and improve coordination is to start with a balloon! Think of it as ball games in slow motion. It gives your child a little more time to react. Try throwing & catching on and off center, hitting the ball up and forward, and kicking too. Once he is doing it consistently, switch to a beach ball, then larger to smaller balls as he improves. Tip Of The DAY!! "W" sitting- This is when a child will sit with their legs splayed out so that if you looked down from above them, their legs form a "W" shape (as seen in this picture). It is commonly seen with children with low muscle tone or weaker trunk control because it creates a wider base of support, making sitting easier. Here's the problem- IT'S REALLY BAD FOR YOUR BABY!! W sitting places baby's hips in a position of internal rotation which can affect gait and posture, create orthopedic issues in her legs as muscles get tighter, and limit trunk rotation in sitting. Here's what to do! If your baby does it occasionally, do nothing- it's fine. But if this is the only way she likes to sit, start fixing her position by placing her in ring/pretzel sitting, her legs straight out in front of her, or even leaning on her arm with her legs out to a side. As she gets older, tell her to "fix your legs". Though she still may do it some of the time when you don't see, getting her used to healthier sitting options will help her avoid potential future orthopedic issues! While the majority of skills in the first 16 months of life happen while baby pushes up off their belly (ie head control, rolling, pushing up in arms, crawling, pulling to stand....) , some do occur while a baby is on their back. One of the cutest 🤡 is when they find their knees and feet. Baby will reach for his knees and feet anywhere from 4-8 months old, with most babies getting their feet into their mouths between 6-8 months old. This skills is important! Tip Of The Day! Happy Friday everyone!! Your kids may be heading outdoors with their friends this weekend. Many will use their tricycle or even bikes with training wheels. But if you don't own one yet, consider a 3 wheeled kick scooter like the ones pictured here. The 2 wheels in front give it a wider base of support to make balancing on it easier. Also, the steering is done by leaning your body so the axle turns in that direction, rather than just turning the handle bars. This makes for a whole body experience that feels natural and is a ton of fun! Tip Of The DAY! Cruising is when children walk sideways at surfaces such as couches and chairs. This skill usually develops between 9.5-11 months. Sometimes, babies even learn to cruise along walls. Tip Of The DAY!! Maximize time spent in the park and outdoors by looking for out of the box activities! Sure, the monkey bars and slides are fun, but that log or giant rock next to the baseball field are a great way to practice balance (with parental supervision, of course😊) and have a blast! I recently took a 2 year old to the park for a session and saw an area paved with cobblestones. He was fine walking on the pavement but as soon as he stepped on those cobblestones, he slowed down and concentrated on not falling! Slowly roll the ball to her and look for her to trap the ball with her hands. She will try to roll it back but will likely not do it smoothly. What is muscle tone and why should I care? Muscle tone is the underlying tension of muscles at rest. Think of it as a default setting in the brain. Kids with low muscle tone usually feel floppy- a rounded sitting posture, and a sloppy walking pattern, among other symptoms. Kids with high muscle tone usually feel stiff- stiff arms and legs, hard to change their diapers, among other things. Here's why you should care: If your baby has low muscle tone, it is harder for them to generate the strength to move and achieve their milestones. Hence, besides the muscle tone, there is usually accompanying weakness, which can complicate development and delay milestones. Tip Of The Day!! Baby's First Shoes!! Every parent bursts with joy seeing baby in her 1st pair of shoes😍😁 But what type of shoe is best? Look for 4 things: 1. High top- A high top shoe gives your baby additional ankle support which helps with early stages of learning balance. 2. Arch support- This puts the foot in the optimal position. Reach into the shoe and feel for a bump, even a small one. Some shoes have stitching to make it look like an arch but it's only foam. Buyer Beware! 3. Flexible- The shoe should bend at the toes (ie. As if standing on tiptoes) so baby can push off as she walks. 4. Sturdy, not flimsy- If you can crush the sole of the shoe and bend it lengthwise, it will be too flimsy. Look for a sturdy shoe that isn't stiff. Of course, always make sure the fit is right. Diagnosis Awareness: Torticollis: Torticollis is a tightness in the sternocleidomastoid (SCM muscle, for short) muscle. When tight, it causes the head to tilt to that side, so the ear is closer to the shoulder, tilts the head forward in upright positions, and the chin is rotated to the opposite side of the tightness, as seen in this picture. When left untreated, it can also cause cranial deformities known as plagiocephaly. It usually presents itself within the first 2 months of life Physical therapy addresses this condition by teaching the family how to stretch the tight muscle and educate the family on positioning. If you suspect that your baby may have this problem, bring it to your pediatrician's attention. Also, make sure to stress tummy time since by lifting his head, your baby will begin to self stretch the tight muscle. Tip Of The Day! Rolling!! I love when babies start to roll! The shocked look on their tiny faces when they roll for the first time is just precious!😊 Babies typically begin rolling from belly to back between 4-5 months, and back to belly between 5-6 months old. Rolling is so important because it's baby's first mobility skill! That means baby can finally get around!! WOOHOO!!! If your baby is 4-5 months old, can tolerate tummy time, but is not yet rolling from belly to back, gently tilt her shoulders and head to one side, with her elbow tucked underneath, as in the picture shown, until the weight of her head initiates the roll. Present toys above her head so she looks toward the ceiling to finish the roll. Give it a try and post your questions and comments. Imagine a line starting at the top of the head, going straight down the nose, chest, belly button, etc. It is important for observing postures and in anticipating milestones.
